HHDCAF - Haxtun Hospital Distributing Cancer Assistance Funds

 

 What is HHDCAF?

HHDCAF is a cancer assistance program that is administered by the physicians at Haxtun Hospital. The purpose of this fund is to assist cancer patients who need assistance with expenses related to their treatment. It is funded by private donations.

 What kinds of expenses qualify?

Expenses such as gasoline, lodging, and prosthesis (such as wigs and breast forms) are examples of expenses that would qualify for these funds. Your physician can help you decide if your request would qualify.

 How do I apply?

You can apply for HHDCAF by completing a short application included with the HHDCAF brochure. Return it to your physician at the Haxtun Family Medicine Center. You will be notified of the level of assistance that will be provided to you. The level of assistance depends on the availability of funds.

 Can I contribute?

HHDCAF welcomes contributions no matter how large of small. The goal is to help meet the needs of community cancer patients while allowing others the opportunity to support one another in times of need.

Contributions may be made in cash or check delivered to Haxtun Hospital or Haxtun Family Medicine Center, or it may be mailed to HHD or HFMC (see address at the bottom of the page). Checks should be made payable to HHDCAF. HHD is a nonprofit entity; therefore your contribution is tax deductible. You can also donate below.
