Haxtun Hospital District, has issued a Request for Proposal ("RFP") for design-bid-build services for the renovation of the Fleming Family Health Center. §32-1-1001(1)(d)(l), C.R.S.

Responses are due no later than:

10:00 a.m. local time, Monday, June 3, 2019, delivered to Haxtun Hospital District, Attn: Joleen Stroyek, 235 Fletcher St., Haxtun, CO 80731.

A Pre-Submittal Meeting will be held:

10:30 a.m. local time, Wednesday, May 29, 2019 on the 2nd floor of the Administration Building, in the Board Room, 235 Fletcher St., Haxtun, CO 80731. Interested firms are encouraged to attend. Attendance is not mandatory; however, Proposers are responsible for any information conveyed at this meeting.

General Statement of Work:

The Haxtun Hospital District is seeking a Design-Bid-Build Healthcare Construction firm to provide a bid for architectural drawings and renovation construction of the Fleming Family Health Center a.k.a. Fleming Clinic Building located at 102-106 W Larimer St. in Fleming CO 80728. §32-1-1804, C.R.S. in compliance with Sections 32-1-1805 and 32-1-1806, C.R.S. as pertains to Integrated Project Delivery ("IPD")

About Haxtun Hospital District:

Haxtun Hospital District is a 25 bed Critical Access Hospital serving the needs of Phillips and Logan County as a Special District. The District owns and operates a hospital and clinic in Haxtun along with a clinic in Fleming.

There is a desire on behalf of the district to support local efforts through the use of local sub-contractors, supplies and labor for this project.
